How Support At Home Works
How the Support At Home Program Works
Australia’s aged-care system is changing. From 1 November 2025, the Support At Home Program has replaced Home Care Packages (transition provisions exist for existing recipients of a Homer Care Package) and provide a simpler, needs-based approach to receiving care at home.
At TSM Seniors, we guide you through each step — from your first enquiry to receiving tailored support from our qualified care team.

A simple, guided pathway to getting the support you need at home.
Step 1
Contact My Aged Care
To access funded support, start by contacting My Aged Care (1800 200 422 or their website). They will ask questions about your health, home situation and the help you may need. If urgent needs are identified, they may fast-track your assessment.
Step 2
Get Your Assessment
A trained clinician will visit your home to understand your daily living needs, mobility and home environment. You will be assigned a Support At Home classification reflecting the type and amount of help you require.
Step 3
Choose Your Provider
Once approved, you can choose a provider that best supports your needs. Choosing TSM Seniors means:

Step 4
Your Personalised Care Plan
We work with you to create an individual support plan within your budget. This includes personal care, nursing, mobility aid, social support, home safety and more. Your routines and preferences guide everything.

Our care team begins delivering services at your home exactly as planned. We keep communication open to ensure everything is safe, consistent and high-quality.
Step 6
Ongoing Review & Adjustments
If your health or goals change, we assist with reassessments. The Support At Home model evolves with you to ensure your support always meets your needs.
